Dual Special Needs Plans (D-SNPs) HMO’s or PPO’s

What is a D-SNP plan and what does it include?

Dual Special Needs Plans enroll individuals who are entitled to both Medicare and Medicaid within their state. There are eligibility categories such as: Full Medicaid (only), Qualified Medicare Beneficiary (QMB) only, without other Medicaid. Please refer to state categories for more listings.

Typically an agent will have to verify eligibility to these special programs. If you qualify for a Dual Eligible Special Needs Plan you may be eligible for additional coverage for Dental, Vision, Hearing, and Prescription Drug Coverage also some Over the Counter Items. Also available with some plans offer: Gym memberships, Healthy food cards, Transportation benefits, and Personal Emergency Response Systems.
